What's an impacted tooth? Akron's own, Dr. Barnett Answers the Question.

In my interaction with new patients, I often (almost weekly) have to explain to a family why one of their teeth will NEVER come in without orthodontic help. This "stuck" tooth, called an impacted tooth, cannot erupt into the mouth because there is not enough room or something blocks the natural tooth eruption.

Wisdom teeth are the most commonly impacted teeth and the usual approach is to remove (extract) them! When it comes to dealing with other impactions though, pulling the tooth is always a LAST resort. Upper canine teeth (eye teeth) are the second most commonly impacted teeth and the most common impacted-tooth-problem an orthodontist has to face. To correct the impaction, it requires a gum tissue surgery and a coordinated approach between an orthodontist and other specialists. The x-ray above and photos below show the hidded nature of an impacted tooth and the progession from the initial surgery to the tooth almost aligned in the arch. As you can see on the x-ray, the upper right side reveals a tooth on an angle crossing behind one of the front teeth. When looking at the photo below, you cannot see the tooth as it is covered with bone and gum tissue--unitl, it's uncovered as shown in the photos at the bottom.

The surgical procedure, called an uncovering, is done during the orthodontic treatment to remove the gum tissue and bone blocking the impacted tooth. Then, the orthodontist can place a bracket on the impacted tooth and slowly move it to the correct position. The total time in braces is usually no longer than an average orthodontic case.

Tooth Brushing Habits Need to Start Young

While most patients are not ready for orthodontic treatment until almost 12 years of age, some early treatment is done in 7, 8 and 9 year old kids. For this reason, tooth brushing habits need to be reinforced at a early age (starting around 2).

Great brushing will not only keep the teeth clean and healthy but with braces, it will prevent permanent stains from forming on the teeth.

While they are young, let your children brush by themselves and then check to make sure they've done a good job. Provide lots of positive reinforcement and give direction on areas that need improvement.

By giving your kids an early exposure to brushing and verifying that their techniques are sound, you're setting the foundation for an appreciation for a healthy and beautiful smile.

Dentists and Orthodontists Need to Work Together

Seeing your dentist every six months is an important part of any orthodontic treatment. Sometimes, it makes the difference in the final aesthetics of the smile.

There's nothing more gratifying than taking off someone's braces and seeing them smile. It's truly an amazing moment and we feel lucky to get to experience it daily at Barnett Orthodontics. Smile after smile, there's nothing better.

There are some patients however, who as part of their treatment plan, don't get that instant gratification the day their braces are removed. Instead, they need to wait on their dentist to cosmetically alter their teeth now that they have been straightened. These cases, require multiple disciplines of dentistry to work together to obtain an ideal result. While often more complicated to work on, it's sometimes even more gratifying to see the final result.

Jeff B's teeth are a great example of one such case. Congratulations on finishing your treatment! What a change...and a special thanks to Dr. Tim Cox for doing an amazing job on the cosmetic dentistry to fix the two front teeth.

Instant Orthodontics? Staight teeth in 6 Months???

Recently I had a patient ask me about an advertisement promising to treat ANYONE with braces in only SIX months. As the old saying goes, if it sounds unbelievable, then it probably is. I suppose it's not a totally false statement, it's just an incomplete promise, an incomplete result and very misleading.

The truth of the matter is that almost anyone's teeth can be improved or straightened rather quickly--in about six months. But to perfect the alignment and to correct the bite, it takes considerably longer. Unfortunately, across the US and on-line (not in Akron Ohio) some individuals are promoting their six month braces as a better option than conventional orthodontic treatment. They promise it is more comfortable, more aesthetic, healthier and most importantly to the consumer, FASTER.

Wanna get rich quick, want to lose tons of weight, regrow all your hair, make money from the comfort of your own home, have braces on your teeth for as little as six months...get the point.

Make sure you're an educated consumer. Do your research and chose wisely.

Extreme Couponing for Braces?

Have you seen the new hit TV show on TLC called Extreme Couponing? It's a documentary series where coupon-clippers 'nickel and dime' their way to outrageous savings, sometimes even FREE cartloads of groceries!

Just like most of you, I get those neighborhood "Value Coupon" packets in the mail with discounts to local carpet cleaners, restaurants, tanning studios, window washers. etc. Extreme couponing, here we come, right?

Not exactly. Usually I just throw the packets away but today I leafed through the coupons and was surprised to find an orthodontist advertising a very LOW 'fixed-fee' for braces. The fee was almost $1000 less than the average cost of orthodontic treatment in Akron Ohio. How could they afford to provide the highest quality of care and have such a low fee? Our office has always tried to keep fees as reasonable as possible. We know orthodontics is a significant expense for most families. It just didn't make sense that someone could "coupon" themselves down to such a low fee.

As one would expect, there was a catch.

The fixed fee for braces did not include the use of any appliances beyond the braces. Thus, there are hidden fees beyond the advertised price. It's kind of a "bait and switch" approach to get you in the door. Is it illegal? No. It's just misleading as you end of paying for expanders, retainers, etc bringing the cost of the treatment up to the local average.

At Barnett Orthodontics, there's no confusion about what the fee covers. There are no additional charges and no hidden fees. No "nickel and diming".

As you can see in the video below, this teen sensation opted for Invisalign Teen -- a clear invisible alternative to metal braces. Only recently did Invisalign become available to teenagers and its popularity is growing based on the trends we see in our practice. We wanted to thank our wonderful patients for donating so much food for the needy over the last six weeks. We are happy to report that we collected over 450 pounds of food for the Akron Canton Regional Food Bank.

On October 15th, our entire office volunteered at the Food Bank and bagged over 1600 bags of cereal. We had a great time giving back to our community.

A special thanks to our patient, Zach Sheppard for joining us on our volunteer day. His mom works for the Food Bank and he generously gave his time to our cause.

Our first annual Mouthguard Day was a terrific success. We met a lot of new people, saw some of our old patients, and most importantly made affordable custom mouthguards to protect a lot of smiles.
While we had a lot of high school athletes come in for a mouthguard, some of the more interesting mouthguards we made were for two of the members of the Rubber City Rollergirls, an all female Roller Derby Team. While their roller derby names may be silly, "MC HamHer" and "Dead Eye HuntHer", the sport is very serious and growing in popularity.

While "creating" smiles is a daily occurrence at Akron's Barnett Orthodontics, this month we hope to be creating smiles of a different sort, on the faces of those a little less fortunate.

In the month of October, our team will be volunteering at a local food shelter, the Akron-Canton Regional Food Bank. We will be helping with the organizing and packaging of their food donations. The food bank helps to feed people and fight hunger by providing food and other essential products to food pantries, hot meal sites, shelters and other hunger-relief programs. We are certain it will make a lot of people smile!

Considering the fact that we have the opportunity to work with almost a thousand area patients, we will be collecting food items for donations to the food bank.

Later this summer/early fall we will be operating a one day only Mouthguard clinic. While Shaq's mouthguard costs almost $3000.00, we will be making you a similar style CUSTOM professional mouthguard at a very affordable price. The Mouthguard Clinic will be open to ANYONE, not just our patients.

Our intentions are to keep cost VERY LOW. It will probably be priced at/near a good boil-and-bite style mouthguard one could get from a sporting goods store. The mouthguard itself, however, will be significantly stronger, better and more protective than a boil-bite-mouthguard. Stay tuned for the date and time.

Braces in Barberton—The Under Appreciated History of the Magic City

Barberton, Ohio, has a rich history dating back to the early 1900s. Despite our practice being located here for almost 30 years, many of our Team Members were unfamiliar with the great history of the city of Barberton. To better educate ourselves, recently, some of us attended the annual Barberton Historical Society Anna Dean Farm Walking Tour. Here are some of the highlights…

Founded by O.C. Barber, Barberton was constructed as an industrial city close to Akron and on the Ohio and Erie Canal. Diamond Match, owned and operated by O.C. Barber was located in the city and was the largest producer of matches in the United States producing 250 million matches a day. On the heels of the success of Diamond Match, O.C. Barber constructed one of the largest and most successful “super-farms” of its time. He mastered the principals of mass production, mechanization, innovation, and long-term planning. The 3,500 acre estate, with a daily operating expense of nearly $1,000, consisted of 12 acres of greenhouses, herds of top-quality livestock, several large barns, a piggery, poultry house, flour mill, concrete factory, brick works, creamery and a 40 acre park. On the highest part of the farm overlooking Barberton laid the 52 room, 50,000 square foot French Renaissance Revival mansion, reportedly described by the New York Times as the “finest mansion between New York and Chicago.” Fully completed in 1910, the mansion alone cost over $400,000 and required 18 months to finish.

Unfortunately the Barber Mansion was torn down in 1965 but many of the Barber Farm buildings have been restored and some are being used by local businesses. There is a wonderful historical society working hard to preserve the history of Barberton and Barnett Orthodontics is proud to be a business in the Barberton community.

The American Association of Orthodontists is sponsoring the “How Orthodontics Changed My Life” video contest. There will be two winners; each winner will be awarded a $2,500 scholarship.

The “How Orthodontics Changed My Life” video contest is open to students who are age 18 or under as of July 15, 2010 and who are legal residents of the United States, Puerto Rico, Guam and Canada. Contest participants must have completed active orthodontic treatment (braces must have been removed) and be in the active retention phase of treatment or have completed active retention. Contest entrants must have received orthodontic treatment from a member of the American Association of Orthodontists.

All entries must be postmarked by July 15, 2010. Videos submitted should be two minutes in length or less. Entries are limited to one per person. There is no fee to enter. Contest entrants should tell their stories on video, answering questions such as:

- What were you like before you had orthodontic treatment?
- How have you changed?
- How proud are you of your new smile?
- When did you notice changes in yourself?
- What kinds of changes have you noticed?
- What does your new smile mean to you?
- What would you say to others whose smiles make them self-conscious?

Before beginning work on your video, carefully review the Terms and Conditions. To submit an entry:

- Burn your video file onto a DVD or save it onto a USB flash drive
- Complete the “How Orthodontics Changed My Life” Video Scholarship - Contest Submission Agreement including all contact information and the signature requested at the end of the agreement form.
- Sign the form if you are 18. If you are under 18, the form must be signed by your parent or guardian
- Mail your DVD or USB drive and your signed submission agreement form, postmarked by July 15, 2010 to:

- Winners agree to participate in public relations announcements by the AAO, including their names, contest video link information and photographs.
- Each of the two winners will receive a $2,500 scholarship to be used toward their post-high school educational expenses at a college, university, vocational training or technical school.

Halloween 2009 at Barnett Orthodontics

For many years now, Halloween has been a day of FUN at Barnett Orthodontics. If you are one of the 60-70 patients who are “lucky” enough to have an appointment that day, you know it can be a great time at the office! Our patients have grown to appreciate the surprise and excitement of what Theme we chose to dress up as each year. For 2009 we decided upon a Peter Pan theme and our office was undoubtedly a NEVERLAND of fun.

From Tinker Bell, to Peter Pan, to Wendy and John, to Mr. Snee, and The Crocodile, The Indian Princess to Captain Hook and his Pirate Posse, we had quite the crew. As you can see in the pictures, we take Halloween seriously. Dr. Barnett was the one handed HOOK—it can be tough to do orthodontics with only one hand.

Many of our patients came in that day dressed in their costumes. Everyone looked so original and creative. We passed out candy to everyone…It’s not often you will get candy from your orthodontist but as Dr. Barnett says, as long as you brush well, you should be able to eat lots of candy—just take it easy on those braces! We look forward to dressing up again next year. Thanks to everyone who made it a special day!

A Great Team Member Leaves For Another Career

We regularly hear that patients at our practice appreciate the familiar and friendly faces of our Team Members at Barnett Orthodontics. It's easy to form a friendship with our Team when patients visit us every 4-6 weeks during a 2 year experience with Braces or Invisalign. Even beyond their time in braces, as patients return for a retainer check, they usually can count on that familiar face to be present in our office.

We are proud of the fact that some of our employees have been with us for over 20 years. There is very little employee turnover, as most people we hire truly enjoy the work they do, the patients they work with and company of their coworkers. When we do lose a Team Member, it is us ally for the right reasons...a baby is born, a relocation to another city, or a dream of a different career path.

Recently, one of our great Team Members had her last day in dentistry/orthodontics. Erin, who had been with us since 2003 has been offered an amazing opportunity to continue her work with children but in the field of education. We are thrilled for Erin and we will miss seeing her on a daily basis. I'm sure many of our patients feel the same way.

CONTESTS KEEP THINGS FUN!

Besides getting a great smile at Barnett Orthodontics, throughout the year we have fun and creative contests with great prizes for our patients. The most recent contest involved a "Mad-lib" where our patients filled in the blanks to a story we created. We based the story on an appointment at the orthodontic office and told them to put in a noun, a verb or an adjective in certain blanks.

We were amazed and entertained by the hundreds of entries. It was difficult to pick the three winners but a few stories stood out from the rest. The passing of a mentor and orthodontic legend--Dr. Gianelly

Dr. Anthony A. Gianelly, Chair ad interim of the Department of Orthodontics and Dentofacial Orthopedics at Boston University Henry M. Goldman School of Dental Medicine, passed away Thursday, May 28. He was 72. "Dr. G" helped to shape hundreds of orthodontists nationwide into motivated, driven, exceptional practitioners. He was known worldwide as a leader, innovator, lecturer and teacher. I had the honor of learning from him on a daily basis during my residency.

Dr. G had a tremendous impact on me and the way I practice orthodontics. He instilled in me the value of evidence based decisions. He was never swayed by the popular fads and while he had an open mind for future developments in the field, he wanted proof that something worked before he would support it.

I feel extremely lucky to have had him as a mentor and I extend my sympathy and support to his family. I am a better orthodontist because of Dr. G.

Toothpaste--a great way to remove Easter Egg Dye!

Like in most homes, dyeing Easter eggs is an annual tradition at the Barnett Household. With a 3-year-old and a 1-year-old it appears that it's just as much fun to dye their hands as it is the eggs. In literally minutes, Annie, our 1-year-old had GREEN hands reminiscent of the "Incredible Hulk".
Regular soap and water proved powerless against the staining. With Easter less than 24 hours away, we needed some quick and potent cleaning power. After a Google search, we learned that toothpaste was great for removing dye stains. Who knew?!? It worked like a charm. I grow to love dentistry more and more ever day...

A Patient at Akron's Barnett Orthodontics on NBC...in the news!

This week on NBC's WKYC-TV Channel 3 news, a story highlighted the struggles of one Akron family's battle with an insurance company from a school sports accident.

Alex Berry, a patient at Barnett Orthodontics has been fighting with the school insurance company for over a year. He was injured playing baseball and lost five front teeth, including all four on his bottom jaw. We have written two letters on his behalf requesting the insurance company approve his case, and finally, via the determination and drive of his mother's efforts, it seems the orthodontic treatment has been granted. We couldn't be happier for Alex. He still has a long road ahead--2 years in braces, dental implants, and crowns, but the financial burden, has at least been significantly lessened. Keep up the good fight Berry Family. We look forward to doing our part to help in restoring Alex's smile to how it was before this terrible school sports accident.

An Orthodontist--Advanced Education Beyond Dental School

A dentist who does braces, is not an orthodontist.

Just as there are specialists in medicine (such as cardiologists, gastroenterologists, neurologists, etc.), there are specialists in dentistry. Orthodontists are dental specialists who dedicate their professional lives to correcting misaligned teeth and jaws. While any dentist can use braces to improve tooth alignment, only an orthodontist has advanced education beyond dental school to learn the proper way to align and straighten teeth.

Orthodontists are qualified dentists, who after graduating from dental school, go on to additional full-time university-based education in an accredited orthodontic residency program supervised by orthodontists. That training lasts at least two academic years—sometimes more. By learning about tooth movement (orthodontics) and guidance of facial development (dentofacial orthopedics), orthodontists are the uniquely trained experts in dentistry to straighten teeth and align jaws.

Annually, there are only 200 orthodontic residency spots nationwide. This makes Orthodontics one of the most competitive and difficult specialities for dentists to get into. Rest assured, by bringing your child to an orthodontist for braces, you are taking them to one of the very best.

How are braces placed on the teeth?

There are two methods of placing brackets on the teeth: direct and indirect bonding.

Teeth are first polished and then prepared with a number of different solutions. The placement of braces requires an extremely dry environment. Therefore, a mouth prop is placed to help keep the tongue and cheeks away from the teeth. A suction tube may also be placed to remove any excess saliva.

In the direct bonding technique, braces are placed one by one, and their positions adjusted in the mouth, prior to being secured to the tooth. This time-consuming process is often completed by dental auxiliaries rather than the orthodontist, as it is usually during busy clinic hours.

Indirect bonding (IB), is an efficient and quick alternative. An impression of the mouth is taken and a model poured. Brackets are then placed on the model and a tray is formed around the teeth and braces in the lab. The braces can be transferred via this tray to the top and bottom teeth all at once.

What are some of the advantages of IB?

• Higher degree of accurate bracket placement than with direct bonding (imagine trying to put a brace accurately on a back tooth inside the mouth compared to putting it on a model where you can move it any way you wish).
• Shorter treatment time and fewer appointments since the brackets are placed more accurately at the first appointment.
• More comfortable and less time spent in the chair for the bonding appointment.
• Often no need for bands, so there is no need for uncomfortable separators
• There is a custom base, allowing the bracket to fit more accurately to the conformation to your tooth.
• The orthodontist is the one who determines the location of each bracket prior to your visit.

Orthodontic treatment can be started at any age. Many orthodontic problems are easier to correct if detected at an early age before jaw growth has slowed. Early treatment may mean that a patient can avoid surgery and more serious complications. The American Association of Orthodontists recommends that every child first visit an orthodontist by age 7 or earlier if a problem is detected by parents, the family dentist or the child's physician.
